The Transitional Services Information Guide is designed for transitional planning. The guide provides information and service providers by grade level. At each grade level there are examples of services, programs and the service providers that may plan or contribute to the student’s transition plan. This guide should not be thought of as a “transition recipe” for all students. Each student’s transition plan should be individualized. Therefore, many options may be considered appropriate. Services and providers will be identified on the transition page of the IEP.
