Steps to Apply for Enrollment at Davey Preschool

Step 1 -FinalForms

Public preschools are required to have on file information regarding enrolled children and their families.  This information must be confirmed or updated yearly. Kent City School District uses FinalForms to collect some of this information.

All forms assigned to a child in FinalForms must be completed as part of the application/enrollment process.

Step 2 - Required Documents

In addition to FinalForms, the following documents must be provided to complete a child's application/enrollment. The documents can be emailed to dbaird@kentschools.net, faxed to 330.676.7435, or uploaded into your child's FinalForms account.

Documents Accepted as Proof of Residency:

  1. Home Purchase/Rental Agreement

  2. Household Utility Bill(s)

  3. Work Record/Pay Stub

  4. Public Assistance Documentation

  5. Voter Registration Card
